  【摘要】目的 通过对人工髋关节置换术患者进行规范化的手术配合，观察人工髋关节置换术程序化和规范化手术配合的效果。资料和方法 对四川省自贡市第三人民医院骨科中心2003年3月～2010年12月诊疗采用人工髋关节置换术的116例患者进行了程序化和规范化的手术配合，术前了解手术方法，充分做好患者、手术间及器械物品的准备，术中根据要求做好全面护理配合。结果 116例病例经术后6～24个月(平均12.3月)随访，Harris 评分表明术后髋关节功能恢复良好，至术后12个月髋关节功能基本恢复正常(术后12个月Harris评分62～93分，平均88.23分。80分以上的89例, 优良率76.7%)。结论 人工髋关节置换术程序化和规范化的手术配合行之有效，能保证术中安全，减少并发症的发生，预防感染，提高手术成功率。

 Normalized operative cooperation for Total hip arthroplasty

 【Abstract】Objective To explore the effect of normalized operative cooperation for total hip arthroplasty (THA) given to patients . Methods In view of the nursing procedure, some normalized measures of operative cooperation, such as Pre understanding of surgical methods, fully understanding preoperative patients, operation and completes instrument, prepare goods required , do a good job according to the requirements of operation with good comprehensive care, were performed in 116 cases with THA in Orthopedics of Zigong third people’s hospital from March 2003 to December 2010. Results Through six to twenty-four months follow-up (the mean follow-up period was 12.3 months), The result of Harris evaluation indicated the 116 cases patients of THA obtained good function recovery of the hip joint after operation and restored their function within 12 months ( The result of Harris evaluation was 62~93 scores. Average 88.23 scores. There are 89 patients' postoperative outcomes were satisfactory, the HHS is more than 80 scores. excellent and good rate were 76.7%). Conclusion The normalized operative cooperation is effective to patients of THA. It is helpful for Surgical safety , it ensure the success of operation, prevent infection and reduce complications.

 人工髋关节置换术可以较快速地重建髋关节功能，由于患者多为老年人，身体抵抗力差，多伴有心脑血管疾病，增加了手术难度和风险，手术的顺利完成依赖于术者的精湛技术和护士的精心配合[1]。我院自2003年3月～2010年12月诊疗采用人工髋关节置换术的116例患者进行了程序化和规范化的手术配合，术前了解手术方法，充分做好患者、手术间及器械物品的准备，术中根据要求做好全面配合。手术经过顺利，无1例出现手术并发症。

 1 资料和方法

 1.1 临床资料 本组选取四川省自贡市第三人民医院骨科治疗中心2003年3月～2010年12月诊疗116例行人工髋关节置换术的患者作为研究对象，男性82例，女性34例；年龄51～95岁，平均65.7岁，其中中老年股骨颈骨折74例，股骨头缺血性坏死37例，人工髋关节置换术后翻修3例，先天性髋臼发育不良2例。住院天数16～30天，平均18.7天，病程最长25年，最短半年。

 1.2 临床表现 髋部疼痛，跛行，活动受限并进行性加重。

 1.3 手术治疗 采用持续硬膜外麻醉或全麻，取90°健侧卧位，髋关节后外侧入路切口，常规消毒铺巾后，将关节囊切开，切除部分关节囊及周围粘连组织，股骨距上1～1.5cm切断股骨颈，取出股骨头，扩大加深髋臼，扩髓，置入假体固定, 检查活动无阻，无活动性出血后，用0.9%生理盐水冲洗，常规放置负压引流，逐层关闭切口。

 1.4 手术护理配合

 1.4.1 术前心理护理 人工髋关节置换术，对病人创伤较大，而且病人年龄偏高，担心手术安全及术后功能，易出现恐惧，焦虑、失眠、等反应。护理人员应参加术前讨论，了解病情，手术体位、方法、器械，以便做好各项术前准备。术前访视时查阅病历，行适当的护理检查 ......
