当前位置: 首页 期刊 《医学新知杂志》 20122 编号：13756137 经脐入路单切口腹腔镜辅助阴式子宫全切手术尝试(1) http://www.100md.com 2012年2月1日 《医学新知·综合版》 20122 经脐入路单切口腹腔镜辅助阴式子宫全切手术尝试,单孔腹腔镜,子宫切除术,阴式【中国分类号】R713.42【文献标识码】A【文章编号】1004-5511(2012)04-   (南方医科大学附属佛山市南海人民医院妇科广东528200)【摘要】目的：探讨经脐入路单切口腹腔镜辅助阴式子宫全切手术的可行性和安全性。方法：经脐部作纵形皮肤切口长2cm，经脐切口下腹方向顶端置入1cm自制穿刺套管并入镜探查，经脐切口上腹方向顶端置入0.5cm自制穿刺套管，超声刀离断输卵管峡部、固有韧带、园韧带后转阴道操作。结果：术中出血约50ml ，手术时间100min。术后无发热等并发症，术后5天出院。结论：经脐入路单切口腹腔镜辅助阴式子宫全切手术是安全和可行的，但操作难度大，需选择合适病例，和良好的腹腔镜及阴式手术操作技巧。【关键词】单孔腹腔镜；子宫切除术；阴式【中国分类号】R713.42【文献标识码】A【文章编号】1004-5511(2012)04-0102-02 【Abstract】Objective: To investigate the possibility and the clinical value of trans-umbilical single port laparoscopic assisted trans-vaginal hysterectomy. Method : A vertical incision of 2cm was made and a laparoscope was put into abdomen at the lower part of the cut, another0.5cm port was set at the upper part of the cut . under the monitoring of laparoscope, the round ligament ,isthmus of the oviduct ,and inherent ligament were cut from uterus with the harmonic scalper, then operation was transformed to trans-vaginal hysterectomy. Result :the operation was successfully performed within 100mins with the blood loss of 50ml.No complication was found during the recuperation. The patient was discharged after 5days.Conclusion: Though it is hard to perform, trans-umbilical single port laparoscopic assisted trans-vaginal hysterectomy is safe ,applicable and valueble, if only right case is selected and excellent skill of laparoscopic and trans-vaginal surgery is available.【keywords】single port laparoscope ,hysterectomy, trans-vaginal 子宫全切手术是治疗子宫病变的常见手术方式 ...... 第 1 2 页 下一页
