  【摘要】目的：观察脑卒中患者病情程度与其抑郁发生率的关系。方法：随机选取我院住院的脑卒中后抑郁症患者300例, 分析脑卒中患者病情程度与其抑郁发生率的关系。结果：轻度、中度、重度脑卒中并发抑郁率呈递增趋势, 患者脑卒中的危重度与抑郁发生率密切相关。结论: 脑卒中患者病情程度与其抑郁发生有相关性。

 To Analyze the Correlation of the Degree of Stroke with the Incidence Rate of Depression of the Patients with Stroke.

 [Abstract]Objective: To analyze the correlation of the degrees of stroke with the incidence rate of depression of the patients with Stroke. Methods: 300 cases inpatients with stroke and depression elected by a random methods were analyzed the correlation of the degrees of stroke with the incidence rate of depression. Results:For the incidence rate of depression, there was an increasing trend from the patients with light degrees Stroke, the moderate degrees Stroke to the serious degrees Stroke. There was correlation between the degrees of stroke with the incidence rate of depression of the patients with Stroke. Conclusions: There was correlation between the degrees of stroke with the incidence rate of depression of the patients with Stroke.

 脑卒中患者由正常人变成生活不能自理，心理压力巨大，可表现出悲观、抑郁、焦虑等心理障碍。脑卒中后最初3个月内明显的抑郁发生率为25-50%，2年后发生率仍高达20-40%[1]。这些心理障碍不仅影响运动及认知功能的恢复，还会影响患者的生活质量。目前，脑卒中患者病情危重度与其抑郁发生之间的相关性研究还不多见。为观察脑卒中患者病情程度与其抑郁发生率的关系，我们进行了如下的研究。

 1 资料与方法

 1.1 临床资料 随机选取我院(2008年至2011年)住院的脑卒中后抑郁症患者300例, 其中男性158例、女性142例；年龄45～87岁，平均65.3岁；病程12.6±5.1W；基础HAMD评分33±4.6分；重度脑卒中96例, 发生抑郁58例，抑郁发生率60.4%；中度脑卒中102例, 发生抑郁42例，抑郁发生率41.2%；轻度脑卒中102例, 发生抑郁31例，抑郁发生率30.4%(见表1)。

 1.2 诊断标准

 1.2.1 脑卒中：符合中国急性缺血性脑卒中诊治指南2010标准。

 1.2.2 抑郁症患者入选标准 ①.符合中国精神障碍分类与诊断标准(CC-MD-3)抑郁症的诊断标准；②.汉密尔顿抑郁量表( Hamilton Depression Scale, HAMD，24项) 评估>20分。

 1.3 统计学处理 采用SAS6.12 统计软件包进行统计分析。脑卒中患者抑郁状态发生危险因素Logistic 回归分析。
