[摘要]目的 探讨个案管理模式在老年糖尿病患者护理中的可行性。方法 選取我院2014年1月～2016年1月收治并定期随访的糖尿病患者138例，按入组编号分为观察组和对照组，各69例，对照组给予常规糖尿病护理方式，观察组给予个案管理模式，比较两组患者的空腹血糖、糖化血红蛋白(HbA1c)水平、糖尿病知识、生活质量及自我行为管理。结果 干预前，两组患者FBG、HbA1c水平比较，差异均无统计学意义(P>0.05)；干预后两组患者FBG、HbA1c水平均有所降低，与干预前比较，差异有统计学意义(P0.05)；干预后观察组DKT、自我行为管理评分各项指标评分有所提高，DSQL评分有所降低，与干预前比较，差异有统计学意义(P0.05)；观察组患者DKT、自我行为管理评分各项指标评分高于对照组，DSQL评分低于对照组，差异有统计学意义(P0.05)；after intervention，the levels of FBG，HbA1c in two groups were all decreased，and compared with those before nursing，the differences were statistically significant (P0.05)；after intervention，the scores of DKT and all indexes in self behavior management in the observation group were increased，the score of DSQL in the observation group were decreased，and compared with those before nursing，the differences were statistically significant (P0.05).And the scores of DKT and all indexes in self behavior management in the observation group was higher than those in the control group，the scores of DSQL was lower than that in the control group，and the differences were statistically significant (P, 百拇医药(孔爱华) 第 1 2 页 下一页
