[摘要]目的探讨胆囊癌的诊断及外科治疗方法。方法回顾性分析46例胆囊癌的临床资料。结果术前影像学提示为胆囊癌的37例手术病例均由术后病理证实。9例非手术病例仅有影像学诊断结果。37例行手术治疗。其中单纯胆囊切除术9例，意外胆囊癌行LC术2例，胆囊癌根治术26例，胆囊癌扩大根治术2例。术后发生并发症8例。8例行放射治疗的晚期胆囊癌患者中5例可见肿瘤缩小。37例术后1、3、5年生存率分别为56.8%、24.3%、10.3%。结论早期诊断，早期手术是提高胆囊癌生存期的关键。B超、CT、MRI等影像学检查是术前诊断胆囊癌的主要方法。根据肿瘤的分期选择适当的手术方式是目前治疗胆囊癌的最主要方法。

 [Abatract] Objective To discuss the diagnosis and the treatment of gallbladder carcinoma. Methods A retrospective study was adopted to analyze of 46 cases gallbladder carcinoma. Results All 37 operation cases of gallbladder carcinoma who had diagnosed by imaging examination were confirmed by postoperative pathology. 9 non-operation cases had only diagnostic imaging finding. There are 9 cases of simple cholecystectomy and 26 cases of radical resection of gallbladder carcinoma and 2 cases of extend radical resection of gallbladder carcinoma in 37 cases of Operation treatment and 2 cases of unsuspected gallbladder carcinoma. Postoperative complications occurred in 8 cases. 5 cases display visible tumor shrinkage in 8 cases of advanced gallbladder carcinoma who received radiation therapy. The survival rates of 1,2,5years in 37 postoperative cases were 56.8%,24.3%,10.3%. Conclusions Early diagnosis and early operation are the key to improve the survival time of gallbladder carcinoma. Ultrasound and CT and MRI are the main method of the preoperative diagnosis gallbladder carcinoma.
