  [摘要] 目的：分析Ⅰ、Ⅱ期乳腺癌保乳手术美容效果的影响因素，探讨保乳术中切缘钟点法取材设计方案，优化保乳效果。方法：回顾我院2003年3月～2008年2月68例Ⅰ、Ⅱ期乳腺癌患者保乳手术中实施钟点法取材，对乳腺癌保乳手术后美容效果的影响进行分析。结果：68例患者随访期限为11～70个月，根据国家“十五”攻关课题“早期乳腺癌规范化保乳综合治疗的临床研究”采用的乳房美容评定标准进行评价：良好61例，占89.71%；一般7例，占10.29%；无一例不满意者。结论：Ⅰ、Ⅱ期乳腺癌保乳手术中，钟点法冰冻这一取材方式能更真实地获取阴性切缘，在保证切缘安全的前提下有效地降低腺体的缺损程度，从而达到最佳的美容效果。

 Significance and cosmetic results of breast conservation therapy by using the method of dial plate excision and frozen section

 [Abstract] Objective: To analyze the factors affecting the cosmetic results of breast conservative therapy to breast cancer patients of stage Ⅰ and Ⅱ, discuss and optimize the method of dial plate excision for breast conservative therapy. Methods: Retrospectively analyzed 68 breast cancer patients of stage Ⅰ and Ⅱ during March 2003 to February 2008used the method of dial plate excision, and then the effect of breast conservative therapy were evaluated. Results: After 11 to 70 months follow up, 61 cases were fine(89.71%), 7 cases were accepted(10.29%), no failure was found. Conclusion: The method of dial plate excision in breast conservative therapy to breast cancer patients in stage Ⅰ and Ⅱ is successful and security, it can not only reduce the excision extension, but also maintain good cosmetic results.

 乳腺癌是女性最常见的恶性肿瘤，由于乳腺癌是一种以局部表现为主的全身性疾病，所以手术范围的无限扩大并不能改善预后。保乳手术是发达国家外科治疗早期乳腺癌的趋势。近10年来，随着诊断技术的发展，国内早期乳腺癌的检出率明显提高，同时，随着广大女性朋友社交活动的增多，对生活质量的要求也越来越高，在希望治愈疾病的同时，更希望保持自己的优美形象。如何使肿瘤彻底切除，又能保持美观，已成为乳腺癌手术的主要研究方向和热点。我院2003年3月～2008年2月对68例Ⅰ、Ⅱ期乳腺癌保乳手术实施钟点法取材，对手术切口、腺体切除范围、残腔处理和缝合方法等诸多因素与术后乳房外形的关系加以分析，取得了满意的疗效和美容效果，现将初步结果报道如下：

 1 资料与方法

 1.1 一般资料

 68例全部为女性，术前常规检查未发现有远处转移，既往无胸壁放疗史、结缔组织疾病和免疫性疾病。年龄29～57岁，平均42.79岁。绝经9例，未绝经59例。患者选择标准：根据肿瘤分期[1](AJCC第6版)，全部患者为Ⅰ、Ⅱ期原发性乳腺癌。肿瘤位于外上象限49例，外下象限11例，内上象限5例，内下象限3例；肿瘤直径1 cm者56例；Ⅰ期26例，Ⅱ期42例。患者肿瘤分期、病理类型及组织学分级见表1～3。
