[摘要] 目的 探讨《肠道病毒71型(EV71)感染重症病例临床救治专家共识(2011年版)》中推荐的临床指标对早期区分危、重型患儿的指导意义。 方法 回顾性分析东莞市人民医院2011年6月～2013年5月收治的EV71感染的第2期和第3期重症手足口病患儿的临床资料。 结果 两组患儿均不同程度出现指南中提出的甄别指标，其中2期组出现上述11项甄别指标中的数量为(3.78±1.61)项，3期组为(6.07±1.55)项，两组比较差异存在统计学意义(P2秒的例数明显高于2期组(P均0.05)。3期组出现2项或以上神经系统表现的例数明显高于2期组(P2秒这5个表现，尤其是呕吐、CRT>2秒和“出现3项或以上神经系统表现”这3个指标最具区分性。

 [Abstract] Objective To explore the guiding significance about clinical indicator in distinguishing critical children from early stage recommended by《Clinical treatment of Expert Consensus involving sever HFMD infected by EV71(2011 edition)》. Methods Various clinical parameters were analyzed in severe HFMD children (phase II， phase III) infected by EV71， hospitalized in Dongguan People’s hospital from June 2011 to May 2013. Results Phase II and phase III groups possessed some clinical indicators suggested by expert consensus inordinately. There were(3.78±1.61) clinical indicators in phase II group and (6.07±1.55) in phase III group. There were significant differences between two groups (P2 seconds in phase III group than that in phase II group (P0.05). There were more cases having two or more nervous system manifestations， especially having three or more nervous system manifestations in phase III group than that in phase II group (P2 seconds which recommended by expert consensus in distinguishing phase II and III severe HFMD. Vomiting， CRT>2 seconds and “having three or more nervous system manifestations” are the most important distinguishing indicators.
