  [摘要]目的:总结臀部及下肢难愈性创面的综合治疗经验。方法:2000 年5 月～2009 年12月,采用简易封闭负压治疗技术、多种皮瓣及皮片移植等整形外科综合治疗方法修复95 例臀部及下肢难愈性创面。结果:除1例电烧伤患者截肢, 4 例截趾外,经1～4次手术其余下肢创面修复后痊愈;对存在数月至数年的下肢慢性创面的患者,应用简易封闭负压治疗技术,创面局部改善后,行皮瓣或皮片移植术,皮瓣、皮片成活良好。术后0.5～2年随访,创面愈合良好,功能恢复满意。结论:加强围手术期治疗,重视整形外科综合技术的应用,臀部及下肢难愈性创面修复可以取得满意的效果。

 Comprehensive treatment of difficult wound in buttock and lower extremity with plastic surgery techniques

 Abstract:ObjectiveTo sum up the experience in repairing difficult wound in buttock and lower extremity.MethodsFrom May.2000 to Dec.2009, comprehensive methods were employed in 95 patients, which included vacuum -assisted closure technology, the skin grafts and skin flaps transfers.ResultsApart from being employed 1 case with amputation and 4 cases with toes amputation, other difficult wounds in lower extremity were repaired and the survival rates were 100%. Vacuum -assisted closure technology were used for the difficult wounds existing from months to years. The skin grafts and skin flaps were used for the wounds when wounds were improved after vacuum -assisted closure technology. Post-operative follow-up for half year to two years showed satisfactory wound repairing and functional recovery.ConclusionsIt might get a good results for difficult wounds in buttock and lower extremity with comprehensive treatment.

 烧伤、外伤、感染、肿瘤,以及糖尿病血管类疾病造成的下肢创面临床常见。由于下肢皮下软组织较少,承受各种压力时,钝性和锐性创伤能够即刻损伤深部软组织及骨,又因血循环差,行走负重等原因,创面愈合较为困难,如有骨关节、肌腱等深部组织暴露,感染则更难愈合。现将我科近10 年来采用整形外科综合治疗方法,修复95例难愈性下肢创面的治疗结果总结报道如下。

 1资料和方法

 1.1 临床资料:2000年5月～2009年12月,共收治下肢难愈性创面95例,102处创面。年龄3～55岁,平均年龄28.5岁。男性57例,女性38例。其中臀部35例,小腿37例,足部23例。致伤原因:撕脱伤8例,火器伤2例,热钢贯通伤1例,糖尿病足13例,褥疮10例,臀部脓肿3例,医源性创面15例,交通伤23例,电烧伤18例,冻伤2例。95例中23例伴有骨外露创面。创面感染细菌多为铜绿假单孢杆菌、不动杆菌、MRSA等。下肢创面存在时间:(635.6±125.8) 天 ,最短15天 ,最长25 年,1 年以上的有54 例。全部创面均有开放感染并有深部组织坏死外露,少数创面曾经外院植皮等治疗,皮片成活不良并形成老化的肉芽组织。

 1.2 围手术期治疗

 1.2.1 所有患者入院后均需行必要的物理检查及辅助检查,进行支持治疗及全身抗感染、抗炎等治疗。控制原发病,如糖尿病患者需降血糖后控制血糖在8～9mmol/L,然后再行进一步的外科处理。

 1.2.2 创面清创,剪除坏死组织, 1.5%过氧化氢与生理盐水反复交替冲洗创面,防止厌氧菌感染。

 1.2.3 创面常规行细菌培养及药敏检查,对耐药菌及少见菌反复检查并针对性应用抗生素局部湿敷创面。

 1.2.4 改良简化的封闭负压创面治疗技术对于糖尿病足部溃疡、下肢静脉淤滞性溃疡创面、深度烧伤残余创面等不适于手术直接修复的创面,采用负压治疗技术,即在创面一端安放引流导管并连接床旁中心负压吸引装置,用粘贴手术巾(江西3L 医用制品集团有限公司生产)封闭,使创面成为密闭的负压环境 ......
