  【摘要】目的 调查精神科住院病历中护理文书存在的缺陷，分析缺陷存在的原因，探讨改进措施，提高护理文书质量和护理质量。方法 组织护理部质控员对我院2010年8月1日-2011年5月31日精神科入档保存病历，按病案号尾数为单数的病历，重新进行质量控制，按湖南省2010年7月1日起实行的新的《病历书写规范与管理规定及医疗质量评定标准》，对其护理文书记录开展质量评价。结果 我院精神科病历中的护理文书，低级错误较多，且存在书写不完整、不准确、不真实等问题，优秀率仅为70%。结论 护士的法律意识淡薄，知识贫乏，工作不扎实是造成护理文书缺陷的主要原因，加强法律知识和业务知识的培训与学习，完善各级质量管理与控制是提高护理文书书写质量和护理质量的对策。

 Psychiatric care instruments defect analysis

 【Abstract】Objective To investigate the medical records of psychiatric in-patient care instrument defects, analyze the reasons for the existence of defects, ways to improve measures to improve the quality of care and quality of care instruments.  2010年7月1日起实行的《侵权责任法》，将医疗责任损害列为第七章，明确了护理文书(记录)是病历的重要组成部分，是以客观资料为依据具有法律效应的文件[1]。同时，护理文书贯穿整个病历的始终，对反映病人的病情、诊治和转归具有极为重要的参考价值，相关规章对护理文书书写有严格的规范要求与质量标准[2]，并对我院护理文书中存在的问题和缺陷进行分析，提出相应改进措施，以达到提高护理质量的目的。

 1 资料与方法

 1.1 资料来源 从我院三个精神科病室中，2010年8月1日-2011年5月31日入档保管的住院病历中，按病案号尾数为单数各抽取120份，共计360份，其中危重病人病历12份，死亡病人病历1份。护理文书内容包括：三测单、长期医嘱单、临时医嘱单、一般护理记录单、危重病人监护记录单、精神疾病患者入院告知书和入院评估、质量记录单等。

 1.2 方法 由护理部组织三个病室护理质控员，对所抽查的360份病历，按湖南省规范要求与质量评价标准[3]，逐一检查，填写自制的表格，并进行分析。

 2 结果

 360份病历中发现有108份存在护理文书缺陷或问题，优秀率仅为70%，其中低级错误(指涂改、错别字，写错病人姓名、性别、年龄、住院天数、入出院时间等)较多，有42份(占11.67%)，书写不完整16份(占4.44%)，书写不准确30份(占8.31%)，书写不真实有12份(3.36%)，与医师书写不一致有18份(5.0%)，同时有9份病历存在2种或2种以上缺陷(2.5%)。其构成比见表1。

 3 结论

 3.1 护理人员法律意识和自我保护意识较弱 由于学校教育和在职教育缺乏法律知识的教育，护士的法律意识和自我保护意识比较薄弱[4]，表现在护理文书书写过程中，态度不认真，作风不严谨，马虎潦草，故而出现涂改，记错病人姓名、性别、年龄、入出院时间、住院天数等低级错误。低级错误的存在，带来较大的医疗风险，曾有一住院病人死亡，因入院日期写错，被患方找到了把柄，医院不得不进行赔偿。
